- Patient Health Data: Medical summaries, oncology diagnosis stages, hospital admission records, and attending physician notes provided voluntarily for treatment subsidy evaluation.
- Donor & Financial Information: Full names, billing addresses, donation amounts, and payment references (payment card details are processed directly via PCI-DSS compliant secure gateways).
- Volunteer & Inquirer Details: Contact phone numbers, email addresses, and professional skills for program placement.
3. How Information is Used
Your personal and health information is strictly used to evaluate medical assistance eligibility, disburse patient grants, issue tax receipts, and communicate regarding care programs. We never sell, rent, or commercialize any patient or donor data under any circumstances.
